This is a normal testis (label says 'sag RT EH', which means 'epidiymal head') with a normal epididymis which is located at the upper end of the testis (to the left on the image, like the head always is for a sagittal image). The square format of the image tells you that this study was done with a linear high resolution transducer, used for small parts of the body and giving the best resolution.
