This is a normal right ovary (label says 'sag rt o') with one cyst that is larger than the others, likely the one that will eventually ovulate. You can tell that it is a transvaginal scan because the sector shape of the view is very wide (blue arrows), almost 180 degrees, and the resolution of the image is very good. Normal ovaries can be hard to find on ultrasound, particularly as patients age.
