This is a right ovary (label says 'Trans Right') with a rounded echogenic region that is consistent with a small dermoid. You can tell that it is a transvaginal scan because the sector shape of the view is very wide (blue arrows), almost 180 degrees, and the resolution of the image is very good. Dermoids are common and benign.
