This is a KUB and it is hard to tell from this single image, but it is actually in the upright position. The kidneys can be faintly seen because the patient has a lot of retroperitoneal fat outlining them. Kidneys are not always visible on a KUB. There is a rather large stone in the upper pole of the left kidney. It is unlikely to move since it is so big, but smaller ones may be present that have given rise to his symptoms in the past.
