“Enlarged cardiac silhouette”

Why don’t we just say “cardiomegaly”?

Because, as in this case, the enlarged contour seen on the radiograph may NOT all be heart!  In her case, she had a huge pericardial effusion by echo, shown here in green.  Pericardial fluid, blood, or tumor can exactly mimic the appearance of cardiomegaly.
