The circle indicates a big box of onions. Did you also notice the basil in the upper right hand corner?

When there is something obvious in an image, we tend to notice it first and then stop looking for anything else. This is called 'satisfaction of search', and can be a big problem when looking at a complicated image like a chest radiograph or CT.
