1. What support lines are present? A right chest tube with tip near the apex and side-hole projecting inside the ribs.
2. What is abnormal? There is an oval well-defined opacity projecting over the region of the right hilum, as well as residual right pleural fluid in a dependent position
3. How acute is the finding likely to be? Given the history, this is not likely acute but any change in symptoms should be elicited
4. What would you do next? Comparison to prior studies, and if the finding is new, chest CT would be reasonable (in this case, the CT showed loculated fluid medially on the right)
