This patient has a large left pneumothorax. The partly collapsed left lung is shown in yellow. This is probably related to the attempted line placement. If you go too deep with the needle when trying to place a central venous catheter, you can pierce the parietal and visceral pleural, letting air leak out from the lung into the pleural space.
