When masses are this large, it is often hard to tell where they arise. This mass seems to be displacing many organs, such as spleen, left kidney, and pancreas, but seems to be separate from each. |

The liver is much lower in attenuation on the CT with IV contrast than the spleen, indicating fatty infiltration. The tumor was an adrenal cortical carcinoma, which can produce quite large masses, as in this case. |
