As previously noted in the tension PTX case, whenever you see a lucent or opaque area on a CXR, check the position of the mediastinum!

Things that can opacify the chest include the following: blood, water, pus, cells, or fibrosis. In this case, there is something blocking the left main bronchus, causing nearly complete left lung collapse.
