In this case, the heart is pulled TOWARD the side of the “white-out”. This tells you that this is a volume-losing process, very different from the previous case of chylothorax. In this patient, the entire right lung had been removed several years ago for lung cancer (a procedure called a pneumonectomy). The heart has gradually shifted into the right hemithorax to take up the empty space.
