Case 3: 63 year old man with shortness of breath

This patient recently had biopsy of upper left mediastinal nodes that were enlarged.  The biopsy revealed lymphoma.  Try to think of several things that could produce this appearance on the chest radiograph.  Assume that the image prior to the procedure was relatively normal.  What do you think of the heart?
