There are double white lines overlying the heart on this close-up view from the lateral radiograph (yellow on labeled image). These are typical of very dense calcification of one of the coronary arteries (the LAD in this case). Calcification must be very severe to be visible on radiography, and is usually associated with stenosis and often angina (as in this patient). For detection of more subtle calcifications, CT is much more sensitive than radiography.
